Seems to me that the cause of this is a lil critter they pick up at the hatchery while very young, and the infected fish then grows to maturity with this deformity. But don't quote me on this. lol

This is straight from ODFW, It's called a stub nose steelhead and its caused by the crazy behavior of the smolt, due to the birds that try to pick them off in the holding tanks. When the smolt sees the bird coming in to snatch the buck or hen away it will start to run into the side of the holding tank walls over and over and thus causing the stub nose. That's my story and i m sticking to it!
